f ）按照机器“安全措施操作方法”如本服务规程的附件一中所示。对实施安全措施的受委托人的签字（第8列）明确说明如下：
f) In accordance with the description of “Operation Regulation of Safety Measure” such as appendix 1 of this service rules, the signature of entrusted person (the 8th column) for implementing of the safety measure is as follows:
f.1) 在实行安全措施之前，安全措施受委托人必须亲自检验所有必要的签字是否都已齐全；
f.1).Before implementing the safety measure, the entrusted person of safety measure shall examine the completeness of all necessary signatures in person;
f.2）只有这样，安全措施受委托人可以对机器或电气设备实施安全措施，并回到保管安全措施登记表的办公室在该表的第8列签字；
f.2) Only in this way, the entrusted person of safety measure may implement the safety measure of the machine or electric equipment, and go back to the office and sign the registration form of the safety measure in the 8th column;
g) 实施负责人在审核了已实施安全措施的人员的签字（第8列），注意到机器或电气设备的安全措施，以及最初该措施的批准后实行签字（第9列）, 明确说明如下:
g) The person in charge of implementation shall sign after verified the signature of personnel who is responsible for the implementation of safety measure (the 8th column ); sign the paper after checking the safety measure of the machine or electric equipment and the earlier approval of this measure (the 9th column ); the description is as follows clearly:
g.1) 在委托承包加工的情况下，按照626/94号法令前第7条款有关协作和合作活动所规定的范围，实施负责人通知承包企业的负责人，必须在以上 h）一节中阐明的登记表中签字;
g.1) In case of consign contracting process, in accordance with the relevantly cooperation and stipulated range of cooperative activity of clause 7 that promulgated earlier of No. 626/94 Decree. The person in charge of implementation shall inform the chief manager of contracting enterprise. It is necessary to sign in the part as the description of h above in the registration form;

h) In case of entrusting to the other enterprise for contracting process, the signature of the person in charge of contracting enterprise (the 10th column ) shall sign according to the signature of person in charge of implementation (the 9th column ), and check the usability of safety measure of machine or electric equipment of implementing theme before implementation.
不可或缺地，在多项服务介入的情况下（如：机械加工车间、电气车间、新设备车间，等的参与）,各相关实施负责人将使用表中的第2, 3 和6列，为委托承包工作，为他的员工实行安全措施申请，必须由承包企业的负责人方面在该登记表的第10列签字。

In addition, as an important task of the safety measure personnel for application are: to verify relevant machines or electric equipment in the registration form; whether or not the safety measure is already implemented by the other previous services, or by the colleague's application: under this situation, the application of this safety measure shall fill in the second or the third column while applying, so as to concentrate the relevant applying of the same machine or the same electric equipment together.
当安全措施受委托人在电气控制箱上的表格中，看到先前已申请并已完成安全措施的某机器或电气设备的重新申请时, 不应在该登记表上实行签名。而必须通知操作技术组长CTE，或车间负责人，由他通知有关该机器和电气设备已申请并已完成安全措施的实施负责人。
As the entrusted person of safety measures sees the earlier application and finish the safety measure of some machine or electric equipment in the form on the electric control cabinet and get another application, he or she shall not sign in this registration form. It is necessary to inform the group leader of operating technology CTE or workshop director to inform the person in charge of implementation of safety measure of that machine and electric equipments.
在该情况下, 实施负责人需要用钢笔划二根斜杠，除去其本人的申请登记，以后将重新按照格式，在前申请多余的空列中填写申请。
In this case, the person in charge of implementation shall draw two oblique columns with the pen to remove his or her register application. And fill in the other column of the form again according to format of application.
5. 机器或电气设备的修复.
5. Repairing of the machine or the electric equipment.
根据采取的措施 (对某机器的日常/临时的检查，保养和清洁，以及对某电气设备或附近的电气装置进行日常/临时维修工作)，对已经实施安全措施的机器或电气设备的修复或重新启动运行，必须按照程序，在同样的安全措施登记表，使用该登记表的右半部分实行书面申请，清楚地填写如下：
a) According to the implemented measure (the daily / temporary inspection, maintenance and cleaning of some machine or electric equipment around, or the daily / temporary repairing of some electric equipment or electric device around), restart the repaired machine or electric equipment which are already implemented the safety measure. In accordance with the regulation of the procedure, it is necessary to use the half part in right of this registration form to apply the same safety measure registration form in writing, and fill in it as follows:
a) 为撤销安全措施申请的日期 (第11列) 和时间 (第12列);
a) Cancel the date (the 11th column) and time (the 12th column) of the safety measure application;
b) 在委托外企业承包加工时，在其本身的职责工作结束时，企业承包负责人的签名(第13列), 具体说明如下：

b.1) 可能发生的情况是：企业承包人在注意到与措施主题有关的机器或电气设备的安全可用性后签了字, 而安全措施本身还未开始使用，在该情况下，对其本身的职责工作完成后的签字，可以在该时刻由负责委托承包工作的某人实行；实施负责人将预防性地在安全措施登记表的“备注栏内写上注解：替代企业负责人”即由企业负责人授权的替代人将在工作完成后在备注内签字；
b1) The possible problem is: Contractor of enterprise signed after checking the machine related to the measure theme or the security usability of the electric equipment, and the safety measure has not begun to use, in this case, the signature of the person on duty after finishing the work is responsible for entrusting somebody for implement the contract at that moment. The person in charge of implementation shall fill in the remark column of the registration form of safety measure preventatively. Substitute enterprise's director, namely, the person entrusted by director of enterprise shall sign in the remarks after the work is finished;
