a)实施负责人的签字（第14列）不管是申请还是撤销与主题有关的机器或电气设备的安全措施, 具体说明如下：
c). Regardless the application or canceling the safety measure related to theme of machine or electric equipment, the signature of person in charge of implementation (the 14th column) is as follows:
c1) 可能发生的情况是：实施负责人申请了安全措施，而未开始使用，这时的修复可以由他的一位受委托人，或由负责当时实行安全措施服务的某位同事实行申请, 无论如何，他将负责当时的机器安全措施，或通过具有机器或电气设备修复责任人资质的操作技术组长CTE或由车间负责人实行。
c1) The possible problem is: the person in charge of implementation applied for the safety measure and has not begun to use; the repairing work at this moment can be applied by an entrusted person or some colleague responsible for implementing the safety measure service at that time, in any case, he will be responsible for safety measure of the machine at that time, or the safety measure shall be implemented through the group leader of operating technology CTE with qualification of repairing of the machine or electric equipment or workshop director .

b.2) 当某人撤销了安全措施，应将机器或电气设备中可以重新使用的信息，通知操作技术组长CTE或由车间负责人；
d2) When somebody has cancelled the safety measure, it is necessary to inform the restart information of machine or electric equipment to the group leader CTE or workshop director;
c) 操作技术组长CTE或由车间负责人的签字(第19列)。 CTE或车间负责人将根据某撤销安全措施人员在(第18列)的签字，并注意到安全措施被撤销，以及机器或电气设备可重新使用的情况后实行签字，具体说明如下：
e.) The signature of group leader of operating technology CTE or workshop director is in the 19th column. CTE or workshop director shall check whether safety measure is cancelled according to the signature in (the 18th column) for the canceling the safety measure. The detail description is as follows:
c.1) 操作技术组长CTE或由车间负责人必须保证各措施授权程序的一致性，因此， 为了机器或电气设备的使用，在检验了所有相关人员，按照与其有关联的内容实行他们本人的签字之前，绝对不应在登记册上签字；
e.1) The group leader of operating technology CTE or the workshop director shall guarantee the consistency of each authorizing procedure of measure, therefore, for the application of the machine or the electric equipment, and before checking all the signature of relevant personnel according to the content of their implementing task, prohibited to sign on the registration book;
c.2) 对产生的所有异常情况，都应直接向自己的负责人说明。
e.2). All abnormal situations shall report to the director on duty directly.
6. 维修操作.
6. Maintenance Operation
对某机器实行的日常/临时检查，维修及清洁措施，根据它们的性质, 需要对该机器进行频繁的启动和停止操作（例如；为更换升降机勺斗、升降机检查、螺杆、链条、传送带、耐热外层的重造、炉子闸瓦的更换，等等），或对于那些在相同的工作中，要求多次重复进行日常/临时维修的电气设备及附近电气装置，如电气设备的供电/断电，因此，必须对机器或电气设备安全措施的相同申请实施限制，在这种情况下，没有任何人能够申请安全措施。
As for daily/temporary inspection, maintenance and cleaning measures for a certain machine, it is required to carry out frequent start and stop operation to the machine in accordance with its properties (e.g. for replacing the bucket of elevator, checking the elevator, reconstructing screws, chains, conveyors and heat-resistant outer layer as well as replacing the brake shoe of furnace, etc.), or for electric equipment and nearby electric devices in the same work, they all need repeatedly daily/temporary maintenance, e.g. power supply on/off for electric equipment. Therefore, the same application for safety measures of machine and electric equipment must be restricted and no one is allowed to apply for safety measures on such circumstance.
尤其是以书面形式对正在维修的机器或电气设备实行的申请，必须由实施负责人在安全措施登记表以清楚的方式或使用红色印章，在第5列-机器字样的下方，与实施措施有关的机器或电气设备识别名称下书写“维修实施中”。
Especially for the applications of machine be maintaining or electric equipment implementation in a written form, the person in charge of implementation must write down “during implementation of maintenance” below the identification name of machine or electric equipment being related to implementation measures in the Line 5- follow the word machine on the Registration Form of Safety Measure in an explicit way or with a red seal.
在委托承包给外企业加工的情况下, 实施负责人有必要通知承包企业的负责人，有关可使用维修服务中的机器或电气设备，并让他在安全措施登记表的第10列中，作为参与小组的直接负责人签字。
In case of consigning contract to other enterprises for processing, the person in charge of implementation must notify the responsible person of the enterprise about applicable machine or electric equipment in maintenance and ask him to sign a name in the Line 10 of the Registration Form of Safety Measure as a directly responsible person in the participating group.
